[r=gmb] Remove the actions menu from a few bugs sub-pages (+activity,
        +addbranch, and +addsubscriber);
        fixes for other sub-pages to follow. Also adds a cancel button where
        appropriate, removes other unnecessary portlets, refines the
        headings, limits form width to a readable maximum, and changes
        the subscriber portlet (only on sub-pages, not the main bug
        page). Fixes: bug 243259: Actions menu still shows up in
        secondary bug pages.
